This is not an actual designer, but a pseudonym used by mug makers in the same way that Alan Smithee is used as a pseudonym by film directors. However, the name was derived from a real person. Lieutenant Colonel "Mad Jack" Churchill (1906-1996) slipped his handmade 14 foot surfboard into the River Severn in Gloucestershire in 1955 and set surfers on a unique path. He had spotted the phenomenon of the tidal bore a massive surge up the river from the sea on Spring tides that creates a rideable wave several feet high. At that moment it is believed Churchill became the first person in the world to surf a river bore. To most, though, Mad Jack is remembered for his fighting Nazis with a bow and arrows and a Scottish sword during WWII...
