Made by Tiki Farm, says "Slap Shot" on the back. Very hard to find.

Perfect condition. No date anywhere, manufacturer, anything except "SlapShot" on the back.

I am not sure what the hole on the front if for. It is embossed on the bottom, "JAPAN" The back has the logo printed "Benihana of Tokyo" Looks to be in perfect condition.
