This double old fashioned was manufactured for Trader Vic's in Waikiki. It is apparently one of six designs based on the menu images. This one features a woman with red flowers in her hair wearing a barrel after loosing her clothes.

Trader Vic's Las Vegas Grand Opening Aloha Coconut. Came in a wooden crate with logo on top and sealed with a "We're Getting Lei'd" sticker. Inside was hay and a Bahai card.
